Dr. Jade Malay, D.C.


Dr. Jade Malay, D.C., is the founder of Apex Physical Medicine and has practiced in the Dallas area since 1992. Her foundational specialty is chiropractic care – which includes neck, back and sciatica pain. The range of specialties is broad, but connected. The root of each condition ranges from lifestyle choices to diet, work, lifestyle, or environment. She finds and treats the cause of problems, not merely the symptoms.

She also leads her industry in a non-invasive treatment for lower back, neck and sciatica pain caused by bulging or herniated discs. For true non-surgical decompression, she uses the DRX 9000 and DRX 9000C, new technologies for gentle, yet effective, results.

Using technology to improve quality of life extends to other areas of her practice as well. She and the medical staff are experienced in the use of lasers for muscle, joint pain, and the burning skin sensation or tingling of neuropathy. She is certified from Bridgeport University in the following areas: mastering functional blood chemistry, functional blood chemistry analysis, and mastering the thyroid. She is experienced in the area of clinical nutrition as it applies to various metabolic issues and its affects on the muskuloskeletal system and the body as a whole. She is a graduate of Lewisville (Texas) High School and proud Baylor University 1988 alumni. She received her Doctorate of Chiropractic degree from Parker College of Chiropractic in 1992. Post doctorate degrees include a diplomate in Chiropractic Orthopedics, of which there are less than 400 across the United States.

Dr. Preston Clay Alexander, MD

Medical Director

Preston Clay Alexander, M.D., is the medical director at Apex Physical Medicine. Dr. Alexander graduated from Southern Methodist University in 1978 then attended medical school at University of Texas Health Science Center in Houston where he graduated in 1983. Dr. Alexander did his post graduate training at Vanderbilt University specializing in Obstetrics and Gynecology. Dr. Alexander currently has a Ob/Gyn practice at Centennial Medical Center in Frisco, TX. In his spare time, Dr. Alexander enjoys traveling and various outdoor activities.


Samer Hamed, D.C.

Dr. Samer Hamed attended Parker University in Dallas, Texas and graduated with a doctor of chiropractic degree in 2011. He has been practicing in the DFW metroplex ever since. He has extensive knowledge in treating automobile accidents, sport injuries, and work-related injuries and has treated many local residents who have been injured. He has vast knowledge in biomechanics and musculoskeletal disorders. He is also fluent in 3 languages (English, Arabic and Hebrew). Dr. Hamed believes that patient care is of the utmost importance and treats every patient like they are one of his family. He will take the time necessary to properly evaluate your condition and formulate a treatment plan specifically for you. He takes great pride in making each patient feel special and his bedside manner is impeccable. He loves to learn more and attends continual education each year to update and provide better quality care for his patients.

Irene Val-Addo, D.C.

Dr. Irene Val-Addo joined Apex Physical Medicine as a functional health chiropractor. Her approach to optimum health combines a thorough understanding of proper structural alignment and movement with clinical nutrition and lifestyle habits to support the whole person.

Dr. Val-Addo earned a Bachelor of Science in Biochemistry from the University of Maryland, College Park going on to work as a laboratory scientist at the U.S. Army Research Laboratory in Adelphi, MD. She co-authored on several research articles and a book chapter on biomimetics. Thereafter, Dr. Val-Addo earned a doctoral degree in chiropractic from Parker University in Dallas, TX. She is passionate about restoring wellness throughout the lifespan. In her spare time, Dr. Val-Addo enjoys spending time with family and friends, doing yoga, and being outside in nature.

Mizi Abiad, FNP-C

Is a nurse practitioner board certified by the American Academy of Nurse Practitioners. She received her Bachelor of Science in Nursing from the University of Oklahoma. She moved to Dallas to work in an Intensive Care Unit with Methodist Health System. At that time, she decided to pursue a field of study that enabled her to provide patient education and improve overall health; thus, she returned to school and received a Master of Science in Nursing degree from Graceland University. She is an active member of the Sigma Theta Tau International Honor Society of Nursing and American Academy of Nurse Practitioners. During her spare time, she enjoys playing with her dog and spending time with her family.

Paige McNamara, PT

Paige has been a PT since 1991. She was raised in Lewisville, TX where she played various sports and went to college on a basketball scholarship. Paige attended Texas Woman’s University and received her B.S. in Physical Therapy. Paige had decided to become a PT when she was 12 years old after watching her father go through physical therapy following hip replacment surgeries and after seeing her favorite great grandfather go through rehab following a stroke. Paige has a passion for helping others and in her spare time volunteers at her church and at a homeless shelter for children. Paige is married with two children and a house full of animals.

Kelsey Glatts, New Patient Coordinator

Kelsey is originally from Philadelphia, PA, and moved to Texas in June 2017. She received her bachelor’s degree in communication from the University of Pittsburgh. Here at Apex, Kelsey coordinates the stem cell and thyroid events in the Dallas/Fort Worth area. When she is not working, Kelsey loves to go on adventures with her dog.

Scott Olls

Scott has been a licensed massage therapist 1999. Scott attended massage school at New Life Massage School in Atlanta, Georgia. Scott has been a team member with Dr. Malay since 2000 and has definitely won the employee longevity award! Scott is the main DRX 9000 decompression technician and has comprehensive knowledge and experience with this machine. Scott also assists with rehab, physical testing, and x-rays. Scott’s passion is baseball. Scott played minor league baseball in his glory days.

Amie Ledezma

Amie has been working for Apex since April of 2016. She has worked in medical administration for the past 15 years in offices ranging from chiropractic to imaging to internal medicine. A Texas native, Aime enjoys spending time with her three dogs, family, and friends. She has dedicated her time at Apex to improving the office environment and the health of our patients.

Tenisha Jones

Tenisha has been a PTA since 2015 and LMT since 2012. Tenisha attended Florida College of Natural Health and Herzing University in Orlando, Florida to complete her schooling for therapy. Tenisha was born and raised in Rochester, NY where she grew up and participated in various sports which led to competing at the collegiate level at SUNY Buffalo State College in track and field. As she recovered from an ACL tear and reconstruction, it was the process of completing physical therapy that helped develop her passion for helping others.

Eboni Gray, DPT, ATC/LAT, LPTA

Eboni is a physical therapy assistant. She received her Bachelor of Science in Athletic Training from the University of Southern Mississippi and recently graduated with a Doctor of Physical Therapy (DPT) degree from Florida A&M University in 2016. She is currently preparing to take her licensure exam prior to practicing as a DPT. Eboni has a passion for sports medicine, pediatrics and orthopaedics. During her spare time, she enjoys exercise and spending time with her family and friends.

Abril Vieyra

Abril decided to become a medical assistant to help out others because she loves to make a change in people’s lives for the better. In her free time she enjoys spending time with her family and friends. She also enjoys dancing and cooking. Her long term goal is to go back to school and eventually become a PA.

Clinton Melerine, LMT / Rehab Tech

Clinton has been a massage therapist since 2011. He is trained on the DRX 9000 decompression machine and MLS laser therapy. He also performs physical testing, massage, x-rays, and helps with rehab. He received a bachelors degree in exercise physiology from the University of Southern Mississippi in 2010. He has done massage and rehab in Mississippi, Florida, and Texas. Clinton loves coaching baseball to his two children in his spare time.

Theresa Jenkins

Theresa is from Forney, TX where she currently lives with her husband and two kids. She has been working as a medical assistant/medical receptionist since 2012. She received her certification for medical assisting from Kaplan College in Dallas, TX. She loves basketball, and spending time with her family.

Chandler Perry

Chandler has been with Apex since 2011. She has an A.S. in kinesiology, nutrition, and health science. She is currently working on her B.A. in kinesiology at TWU. Her ultimate goal is to be a physical therapist and continue her work in helping others.

Diana Lopez

Diana has been with Apex since 2015 as an office assistant. She is currently working on her B.A. in sociology at the University of North Texas. In her free time she enjoys spending time with her family and friends.
